Full Job Description
The Support Worker will deliver dedicated in-home care to individuals with disabilities, helping them lead fulfilling lives and achieve independence. This role involves not only practical assistance but also emotional support to enhance clients' well-being. You'll take part in planning and implementing personalized care strategies, assisting clients in achieving their goals, and encouraging participation in community and social activities. The Support Worker will also communicate effectively with families and healthcare professionals to ensure seamless care, identifying any additional support needs as they arise.
The Work You'll Do:
- Advocate for clients’ needs and rights in the health and social care system
- Assist clients with personal care, daily activities, and skill development
- Collaborate in developing and implementing individual care plans to meet client needs
- Document and report any changes in the client's condition to the relevant authorities
- Encourage and facilitate the client's participation in community and leisure activities
